Delete a qualification's supporting document

Removes the document entry and its stored file. Use it to clear an entry whose upload failed and will not be retried, or one created by mistake — nothing is removed on a timer, so an abandoned entry stays until it is deleted.

Works on a PENDING entry as well as an UPLOADED one. 404 if the qualification or document is unknown.
